The Case in Point Platinum Awards recognize population health management programs that successfully deliver efficiency and effectiveness across a variety of disciplines and settings in the continuum of care coordination. Nurtur was recognized in the Disease Management/Population Health Programs category.

Members are proactively identified and stratified into the appropriate level of intervention--including home visits as well as educational materials that are culturally sensitive and leverage health literacy best practices. Medication management also improved, as compliance with controller medication prescriptions increased while rescue medication prescriptions dropped significantly.
