EMIS has developed a fully integrated risk stratification solution using the peer reviewed and clinically-validated QAdmissions algorithm. The embedded tool will case-find patients before they have a secondary care admission enabling GPs to classify their risk factors and create care plans. Practices will be rewarded for achieving NHS England's Enhanced Service (ES) by identifying and proactively managing these patients with personalised care plans.

The risk stratification solution will help:

  • GP practices to score patients both en masse and individually, allowing for population profiling and face-to-face scoring during consultations. Calculations are run within EMIS Web so patient information does not need to be extracted.
  • CCGs to see a view of patients who have been risk stratified via EMIS Enterprise Search and Reports (with the appropriate sharing agreements in place) to support health service planning and commissioning.

Both practices and CCGs will be supported with a suite of EMIS Web decision making tools to manage the top 2% of patients at risk of a hospital admission. Data Quality Toolkit searches identify patient records that can be enriched to improve the accuracy of risk scores. Patient Management support patient outcomes with a case management register and care planning. Practices can also review their case management register with ease on a monthly basis - as recommended in the ES.
