Humana Inc. (NYSE: HUM), a leading health and well-being company, today announced it has collaborated with CHI Franciscan Health on an individual-focused model of health care for Humana Medicare Advantage members in Washington.

Known as value-based care, the model is designed to help people achieve their best health by emphasizing:

  • More face time with physicians and personalized care that is tailored to each person’s unique health situation;
  • Access to proactive health screenings and programs that are focused on preventing illness;
  • Improved care for people living with chronic conditions with a focus on avoiding health complications (watch Humana’s “Better Chronic Management Through Value-Based Care” video);
  • Leveraging technologies, such as data analytics, that connect physicians and help them work as a team to coordinate care around the patient; and
  • Reimbursement to physicians based primarily on the health outcomes of their patients rather than solely on the number of services they provide (fee-for-service).

Humana’s Medicare Advantage plan members will now have in-network access to CHI Franciscan Health. In addition to offering patient services, the collaboration will focus on improving specific health metrics in Washington that are associated with high-quality care.

Under this new agreement, CHI Franciscan Health’s physicians will also benefit by having new tools, access to real-time data and access to care support to maximize time spent with their patients covered by Humana Medicare Advantage.

As of Sept. 30, 2017, Humana has 1.9 million individual Medicare Advantage members (out of 2.8 million total individual MA members) and 148,700 commercial members who are cared for by 51,500 primary care providers, in more than 900 value-based relationships across 43 states and Puerto Rico.

As of Sept. 30, 2017, Humana has reached its calendar year goal of having approximately 66 percent of Humana individual Medicare Advantage members in value-based payment relationships. Humana’s total Medicare Advantage membership is approximately 3.3 million members, which includes members affiliated with providers in value-based and standard Medicare Advantage settings.
