UnitedHealthcare’s Connected for LifeSM online care-management resource is now available free of charge to caregivers who help manage the health of family members enrolled in UnitedHealthcare Community Plan of Kansas Medicaid plans.

Connected for Life includes a secure website and mobile app where caregivers can manage a calendar, manage medications, check vital signs, record health events and store personal health information for individuals who are disabled or receiving long-term care services. The website and app include information and links to resources that help people find helpful community resources.

Family members who serve as caregivers provide critical services and support, often delaying or preventing hospitalizations, readmissions and nursing home placements. However, nearly 70 percent of family caregivers report their primary source of stress is related to caring for a loved one, according to the AARP Public Policy Institute.

“Caring for a loved one can often be an overwhelming task, and many caregivers are finding it increasingly stressful and challenging to juggle their caregiving responsibilities with their personal lives. Connected for Life can help alleviate some of the stress they face by providing a secure and centralized resource to help them more effectively and conveniently manage their caregiving duties, and connect them with community resources to provide the help they and their loved ones may need,” said Tim Spilker, CEO of UnitedHealthcare Community Plan of Kansas.
