UnitedHealthcare, a UnitedHealth Group (NYSE:UNH) company, introduced a new Medicare Advantage plan in the Jacksonville area that will feature a medical home model of care management. WellMed, a health care delivery system that specializes in the complete coordination of care for seniors, will coordinate the care for beneficiaries who enroll in the new plan.

The more than 169,000 Medicare beneficiaries in Clay and Duval counties can enroll in the AARP MedicareComplete Focus (HMO) Medicare Advantage plan during the Open Enrollment Period (Oct. 15-Dec. 7).1

The plan is designed to help people save money and improve their health and well-being through additional benefits and health care services beyond those available with Original Medicare.

The AARP MedicareComplete Focus plan has a $0 monthly premium beyond the Part B premium, and includes coverage of prescription drugs, a SilverSneakers fitness membership and UnitedHealthcare’s Passport program, which gives consumers access to their benefits when they travel outside of the plan’s service area.

The plan covers hearing aids, routine vision and hearing care as well as an annual physical – above and beyond the annual wellness visit available through Original Medicare – to encourage members to build relationships with their primary care physician and get recommended screenings and preventive care services. Members can visit their primary care physician for a $0 copay.

To supplement the care from their primary care physician, members can receive a physical exam and comprehensive review of their health status in the comfort of their home through UnitedHealthcare’s HouseCalls program. Beneficiaries can also reach health care professionals 24 hours a day, seven days a week, through the NurseLine for support with urgent health issues or concerns that can’t wait for the next available doctor’s appointment.

WellMed will remain in the network for people enrolled in the AARP MedicareComplete Choice Plan 2 (Regional PPO) Medicare Advantage plan, which UnitedHealthcare will continue to offer in 2015.

AARP MedicareComplete Focus members will have access to a robust network, including more than 230 primary care physicians, more than 1,100 specialists and eight hospitals in the Jacksonville area.*

The plan’s network is designed according to a medical home model, meaning WellMed will coordinate plan members’ care to help ensure they receive the right care at the right time in the most appropriate setting.

WellMed is a group of employed and contracted primary care physicians, specialists, nurse practitioners, physician assistants, registered nurses and other medical staff who work as a team dedicated to high-quality, efficient and effective health care. WellMed’s model of personalized, coordinated care places particular emphasis on preventive care and is specifically designed to meet the needs of seniors.
