Conifer Health Solutions said today that Megan North, president of its Value-Based Care business line, and James Slaggert, vice president of Integrated Health Networks for Catholic Health Initiatives (CHI), will co-present at the 2014 American Hospital Association (AHA) Leadership Summit. Their session, entitled “The Journey to Value-Based Performance,” will focus on the steps hospitals and health systems must take to create an enterprise-wide roadmap to guide their shift to value-based care delivery.

Slaggert and North will discuss CHI and Conifer Health’s partnership, and their work to develop the blueprint to achieve clinically integrated networks in key markets. They will also recount the milestones that set the stage for CHI’s multi-phased approach to population health management, hospital and physician alignment; and ultimately, to becoming an Accountable Care Organization (ACO).

North and Slaggert will deliver “The Journey to Value-Based Performance” on July 22 at 8:30 a.m. The AHA Leadership Summit will be held July 20-22 at the Manchester Grand Hyatt in San Diego.
