UnitedHealthcare Community Plan has expanded its service area to include four additional New York State counties in which it offers the New York State government-sponsored health insurance programs - Medicaid Managed Care and Family Health Plus.

UnitedHealthcare's New York State government-sponsored Medicaid Managed Care and Family Health Plus programs are now available in New York City, on Long Island, in the Hudson Valley and throughout Central and Western New York.

The 38 counties - which now include Lewis, Ontario, Seneca and Wayne - are: Albany, Bronx, Broome, Cayuga, Chautauqua, Chemung, Chenango, Clinton, Columbia, Essex, Fulton, Genesee, Herkimer, Jefferson, Kings, Madison, Monroe, Nassau, New York, Niagara, Oneida, Onondaga, Orange, Oswego, Queens, Rensselaer, Richmond, Rockland, St. Lawrence, Suffolk, Tioga, Ulster, Warren and Westchester.

UnitedHealthcare Community Plan offers programs for adults and children who may qualify for Medicaid and Child Health Plus. These programs offer comprehensive health coverage, including preventive care, primary care, hospitalization, prescriptions and other services - often with little or no cost.

Members of UnitedHealthcare Community Plans have access to various wellness programs, including nurse outreach and support for members who may need additional assistance in managing chronic diseases, and the "Healthy First Steps" program for expectant mothers and newborns. Members also will receive additional support through UnitedHealthcare's member-outreach programs to remind and encourage them to seek preventive care.

"We continue to expand UnitedHealthcare's service area and wellness programs so more eligible New Yorkers and their families can access affordable, quality health care, particularly preventive care," said Pat Celli, president of UnitedHealthcare Community Plan of New York. "We look forward to engaging with local health and other community-based organizations as we strive to help improve the health of our members and our communities."
