The new agreement expands upon the parties’ existing network contract, which will continue to provide Humana Medicare Advantage members with access to a wide range of health care services at Einstein Healthcare Network hospitals, outpatient care centers, and physician practices in Philadelphia and Montgomery Counties. The value-based care agreement, in turn, teams Humana with Einstein Healthcare Network’s Accountable Care Organization, Einstein Care Partners, toward enhancing both the patient experience and patient outcomes in the Philadelphia area.

“We consider value-based care programs an important element to payor-provider relationships, as we continue our mission to improve population health outcomes and reduce health disparities,” said Einstein Care Partners Executive Director Chris McTiernan.

“We are pleased to work with Humana on this value-based care initiative, as we share a vision in providing high-quality and cost-effective care for the communities we serve,” said Einstein Care Partners Medical Director for Population Health Shiva Chandrasekaran, M.D.

The value-based care model emphasizes:

  • More personal time with health professionals and personalized care that is tailored to each person’s unique health situation;
  • Access to proactive health screenings and programs that are focused on preventing illness;
  • Improved care for people living with chronic conditions with a focus on avoiding health complications (watch Humana’s “Better Chronic Management Through Value-Based Care” video);
  • Leveraging technologies, such as data analytics, that connect physicians and help them work as a team to coordinate care around the patient; and
  • Reimbursement to physicians linked to the health outcomes of their patients rather than solely on the quantity of services they provide (fee-for-service).

“Humana is delighted to expand our ability to offer value-based care to our Medicare Advantage members in and around Philadelphia,” said Humana Regional Medicare Vice President Eric Bohannon. “This agreement further unites us with Einstein Healthcare Network as together we intensify our efforts to provide access to care that improves the well-being of Pennsylvanians.”

Humana has an extensive and growing value-based care presence. As of September 30, 2017, Humana has 1.9 million individual Medicare Advantage members (out of 2.8 million total individual MA members) today and 149,000 commercial members who are cared for by 51,500 primary care providers, in more than 900 value-based relationships across 43 states and Puerto Rico.
