CHE- 109.290.158 - a shareholder of FABRYKI MEBLI "FORTE" S.A. based in Ostrów Mazowiecka (the Company), I propose Mr Maciej Formanowicz as a candidate for Member of the Supervisory Board of the Company.

Maciej Formanowicz is the founder of FABRYKI MEBLI "FORTE" S.A. He has served as President of the Management Board since 1993. As of 31 August 2023, Maciej Formanowicz resigned from his position on the Management Board of the Company.

He is a graduate of the Faculty of Wood Technology at the Higher School of Agriculture in Poznań. Mr Formanowicz began his professional career in 1972, holding positions as a technologist, manager of the Development and Investment Department, laboratory manager and department manager at the Wyszków Furniture and Interior Design Factory, in north-eastern Poland. From 1978 to 1981, he was the technical director of FORMAT Sp. z o.o. and from 1981 to 1987, he was the CEO of HASTA Sp. z

1. based in Oborniki Śląskie. In 1987, Maciej Formanowicz moved to Wrocław where he took the position of deputy director of MAKRO Sp. z o.o. In 1991, he was appointed director of the MF corporation. From April 1993, Mr Formanowicz was President of the Management Board of FORTE Sp. z o.o., which was transformed into FABRYKI MEBLI "FORTE" Sp. z o.o. and then into FABRYKI MEBLI "FORTE" S.A. Mr Formanowicz served as President of the Polish Association of Furniture Manufacturers from 1996 to 2009. Mr Formanowicz also holds many professional awards.

I, the undersigned Maciej Formanowicz, express my consent to stand as a candidate and serve as a Member of the Supervisory Board of a company operating under the business name of FABRYKI MEBLI "FORTE" Spółka Akcyjna based in Ostrów Mazowiecka (no. KRS 0000021840).

I declare that I meet all the criteria needed to stand as a candidate for Member of the Supervisory Board of FABRYKI MEBLI "FORTE" S.A., in particular the following:

• I havefull legal capacity and there are no legal obstacles for me to serve as a Member of the Supervisory Board, in particular there are no obstacles listed in Article 18(1) and (2) and Article 387 of the Code of Commercial Companies;
• I am involved/ I am not involved in any activity competitive* to FABRYKI MEBLI "FORTE" S.A. based in Ostrów Mazowiecka and I participate / I do not participate* in a competing company as a partner in a civil law partnership or any other partnership or as a member of a governing body of a competitive capital company or a member of a governing body of any competitive legal entity;*

6. Recipients of your personal data may include competent authorities acting in accordance with applicable laws. In addition, your data may be transferred to entities contracted to process personal data on behalf of the Controller, including IT service providers, in which case the data is processed by those entities on the basis of a data processing agreement made with the Controller and only in accordance with the instructions provided by the Controller.
7. Your data will be processed for the duration of your election as a member of the Supervisory Board and once the election process has been completed, until the statute of limitations for claims under generally applicable laws has expired.
8. You have the right to access your data and the right to rectify, erase, restrict processing, the right to transfer data and the right to object to processing.
9. If you believe that the processing of your personal data violates the GDPR, you have the right to lodge a complaint with the President of the Personal Data Protection Office (Prezes

11. Providing your personal data is voluntary but necessary to carry out the election of a Member of the Supervisory Board. Failure to provide your personal data will prevent you from participating in the election of a Member of the Supervisory Board.
